(This form must not be expired and must include all required shots for child's age)

2. Good Health Certificate, form #3040
(This form is valid for 2 years from the date of the last physical)

These forms can be obtained from your child's pediatrician
(Out of state or country forms are not accepted)

Forms are due on the first day of attendance
